Covid19 Vaccination Update

The Kanawha Charleston Health Department will be onsite at Edgewood Summit on Tuesday, December 15 thru Thursday, December 17, to administer COVID vaccine to our Residents and Employees according to the schedule which follows.  It is very important to have read the COVID Fact Sheet which was distributed to all today and complete and sign the Consent Form.   Bring the completed Consent Form with you to be vaccinated.

At your Vaccine Administration time scheduled for your Wing and Floor, please report to the Independent Living Dining Room.  You will have a chance to read the Vaccine Fact Sheet and complete your Consent Form in the Dining Room if needed.  Next, you will be instructed to go to the Multipurpose Room to get your vaccine.  Finally, you will be asked to wait for 15 minutes in the Library in case you have any adverse reaction.  Then you are free to return to your apartment.  Staff will be available in each area to assist you if needed.

The Vaccine Administration Schedule is below.  It is very important to be on time or early for each time slot.  The vaccine has a limited number of hours to be given after thawing.  You must have previously said YES to receiving the vaccine.  If you said NO or MAYBE, your next chance to receive the vaccine onsite will be in 21 days.
